2021 Member Checklist

CampDoc Medical Form

You should have been emailed a link to fill out the CampDoc medical form. If you didn’t receive a link, please email Brad Furlano to troubleshoot.

Items needed to complete Medical Form:

  • Completed Sports Physical (click to download form)

  • COVID Vaccination Card (after final shot)

  • General Vaccination Completion Schedule (as you would provide a school)

  • Drivers License or State ID

If you are a minor, a parent or guardian will automatically be invited to your CampDoc profile. If you would like to add another; or if you are 18 or older and would like to add a parent/guardian to assist with completing the form:

Transportation to Forest City

How are you getting to Waldorf University on July 1?

Once you know how you’re getting to Forest City, fill out this form so we know your arrival method and time.

You must email and request an exception with Russ if you are arriving after July 1 or are leaving and coming back during the season.

Season Arrival and Departure Information for flights

Arrival: Thursday, July 1

Fly into MSP

Arrive by 7pm at the latest.

Departure: Sunday, August 15

Leave from IND

Can schedule to leave early AM. We will provide transportation to the airport. Later flights will mean more time spent at the airport.

Any transportation to or from the corps not on July 1 or August 15 needs to be cleared with Russ Gavin ahead of time.
